Vet Life Benefits offers Veterans seeking Benefits the Quality of Life Assessment (QOLA) along with our Quality of Life Assessment form that shows how the QOLA correlates with the Department of Veteran Affairs Review PTSD Disability Benefits Questionnaire.  

The QOLA Profile along with the correlating form adds valuable evidence to a veteran's benefits package.  It is easy to read and understand.  ASSESSMENT TESTIMONIES

RESULTS OF THE QUALITY OF LIFE ASSESSMENT PACKAGE MUST BE SENT TO A DOCTOR OR ATTORNEY WHO IS WORKING WITH THE VETERAN TO COMPLETE THEIR BENEFITS PACKAGE.   WE WILL NEED THE NAME, PHONE #, AND EMAIL ADDRESS OF BOTH THE VETERAN AND THEIR PROVIDER.  A donation of $75.00 is requested to help our organization continue to assist veterans and their families.  Donations are tax-deductible.  DONATE TO VET LIFE BENEFITS

QOLA SOFTWARE - COMPUTER-GENERATED PERSONAL PROFILE

Life Renewed Quality of Life Assessment software assesses these dimensions: Physical Health, Physical Limitations, Body Pain, Energy Level, Health Behaviors, General Health Perceptions, Emotional/Mental Health, Emotional Limitations, Social Functioning, Social Compatibility, Life Experiences, Life Purpose Index, Support System, Stress Index, Spiritual Beliefs, Situation Resolution and provides a depression index. When the assessment is completed, it is automatically scored and a personal Quality of Life Profile is generated. The assessment profile is the blueprint for understanding the strengths and challenges faced by clients.

The Life Renewed Quality of Life Assessment Software was designed for organizations that help individuals achieve a better quality of life. It is for individuals ages 18 and over and is not gender specific. The assessment takes approximately 30 minutes to complete. The multiple- choice questions are easy to understand and answer. This assessment is designed to plug into your process. The personal profile is easy to interpret.

HOW DOES THE QOLA COMPARE WITH THE DEPARTMENT OF VETERAN AFFAIRS REVIEW PTSD DISABILITY BENEFITS QUESTIONNAIRE?

 

Quality of Life Assessment (QOLA) Scoring: the client receives a percentage score for each module.

  • Over 55% - above average
  • 45-55% - average
  • 35-45% - below average
  • Under 35% - significantly below average
  • Percentage scores are computer-generated.

 

Department of Veteran Affairs Review PTSD Disability Benefits Questionnaire Scoring:

  • A Checklist / Brief narrative is used to record responses.
  • Responses are hand-checked and recorded by the interviewer.

 

 

 The QOLA is done on a computer so no interaction with the interviewer might affect a response.  Additionally, the QOLA is more comprehensive and assesses some qualities of life not included on the VA PTSD Disability Questionnaire.

HOW MUCH DOES IT COST?

We do ask for a $75.00 donation for each Assessment.  Life Renewed is a 501(c)(3) tax-exempt, nonprofit organization, so you will get a receipt that can be used for tax purposes. All donations go to helping our organization meet the needs of our program and the veterans we serve. The name of the Life Renewed program that serves veterans and their families is Operation Not Forgotten.
